Nobody will ask for your last name. Nobody will sign you up for anything or put you on a list. There is no cost, ever. If you want to stop drinking, that's the only qualification there is.
If you're not sure whether you belong here, come to an open meeting and listen. A lot of us did exactly that before we decided anything.
